What is Maisha Health Fund?

Maisha Health Fund, formerly Steward Health is the health insurance business under the Cassava Smartech group.

Choose a medical aid plan

We have medical aid plans for individuals and groups as follows:

For Individuals For Corporates/ NGO, SMEs For Tertiary Institutions ( colleges & universities) For Schools
You can join as a family or as an individual Minimum of 4 principal members

 

Two options:

Corporate scheme– traditional medical aid

Corporate Fund– self-insurance.

Paid as part of tuition fees per student per semester

–        Covers semester period and holiday

Paid as part of tuition fees per student per term

 

–        Covers students during the school term only

What is the difference between a Corporate Fund a traditional medical aid cover?

  1. A corporate fund is a self-insurance product. Maisha Health Fund sets up a Fund for the Corporate’s staff and their dependents. Corporate enjoys active participation in the administration of the Fund through setting up rules governing the administration of the Fund. Funds belong to the corporate.
  2. Traditional medical aid cover means that funds are contributed into a pool owned by the medical aid society.

How much will be my monthly contribution?

Monthly premium costs differ as per the package you choose and number of dependents covered as per table above

How do I register to become a member?

You may sign up online or download the application form (link to the form), complete it and submit via email to sales@maishahealthfund.co.zw

Submit form together with the following:

  • Copies of national ID (for adults)

 

  • Copies of birth certificate (for children under 18)

 

  • Proof of school attendance for children above 18 but in school

 

  • Passport photos for each member

 

  • Proof of residence

Which service providers accept Maisha Health Fund cards?

Our card is widely accepted at major public and private hospitals, surgeries and pharmacies nationwide. A comprehensive list is available on the service providers’ portal on the website.

  • Maisha Health fund may sign up new service providers as per member’s request

What is the age limit for joining Maisha Health Fund?

Maximum allowable age limit – 75years if you were under medical aid otherwise below 65years

How do I make a refund claim?

Download claim form on the website, complete it and send it via email to claims@maishahealthfund.co.zw. Send it together with valid receipts, (drugs prescription, pharmacy claim form if it’s for pharmacy) and claim form for services received.

How do I become a Maisha Health Fund service provider?

Requirements: Complete a Maisha Health Fund Service Level Agreement and submit together with copies of the following documents

    1. Health Professions Association (HPA) Certificate / Practicing
    2. AHFoZ Letter/Certificate
    3. Current Tax Clearance Certificate
    4. Banking details ((both FCA & ZWL) on Letterhead and signed for

How it works

You can register, add dependents and pay for your medical aid without leaving the comfort of your home.

You can select from our six packages and register for yourself or your whole family.

How do I know if I have fully registered?

When you complete your registration, you will receive a confirmation message. You will also be advised your medical aid number which you will use to access health services.

How do I get my medical aid card?

You will be contacted by our membership team who will ask you to share particulars of your dependents so that cards can be produced for them. You will also be advised to do a biometric registration.

How do I access my medical aid?

You will require biometric enrolment of your and dependents’ fingerprints to authorize treatment authorisation and claim submissions that require fingerprint validation with enabled Service Providers.

Where can I do the biometric registration?

You may visit your nearest health service provider who will scan your fingerprints to register you.

What are benefits do I get from my medical aid cover?

Who can I add as a dependent on my medical aid?

You can add your spouse, parents, children, mother or father in-law or any relatives as many as you can pay for.
